Our Approach to Aged Care
Our team supports all aspects of a client’s care needs through a multidisciplinary, collaborative approach. This may include assistive technology, home modifications, and functional assessments, tailored to each individual’s goals and environment.
If you don’t have in-house allied health staff, or require additional support, First Choice Allied Health can help. We provide timely access to a pre-qualified, experienced allied health team, to better support providers in delivering comprehensive clinical care that is aligned with the Support at Home program guidelines and aged care quality standards
Services Offered - Clinical Allied Health Services
Occupational Therapy
Our Occupational Therapists support older adults in being able to live safely and independently at home. We provide comprehensive assessments and practical, tailored interventions to assist with mobility, personal care, home safety, cognition, and daily activities.
Our services are commonly delivered through the Support at Home program, helping individuals remain independent in their own homes for longer.
Speech Pathology
Our Speech Pathologists support older adults experiencing difficulties with communication, swallowing, voice, or cognitive communication. We provide comprehensive assessments and therapy to assist with speech clarity, memory and language changes, and safe swallowing.
Our services are tailored to support improved health, dignity, and participation in everyday life at home.
Psychology
Our psychology services support older adults experiencing challenges such as anxiety, depression, adjustment to health changes, grief, or social isolation.
Our team brings a holistic approach, working in collaboration with clients to build coping strategies, improve emotional wellbeing, and support mental health during periods of change.
Services can be delivered at home or via telehealth where appropriate, depending on the individual needs of each client.
Assistive Technology and Home Modifications
Assistive technology and home modifications can significantly improve safety, independence, and mobility for older adults living at home. Our clinicians assess each individual’s needs, alongside their home environment, in order to provide tailored recommendations and equipment solutions, including mobility aids, bathroom modifications, ramps, rails, and adaptive devices.
All recommendations are practical, evidence-based, and aligned with aged care funding pathways including the Assistive Technology and Home Modifications (ATHM) Scheme.
Home Safety and Falls Prevention
Falls prevention is an important part of maintaining independence for older adults living in the community.
Our team provides comprehensive home safety and falls risk assessments to identify environmental hazards, mobility challenges, and functional risks. We offer practical recommendations, equipment solutions, and home modifications to reduce risks while supporting safe independence at home.
Restorative Care
Our restorative care services focus on helping older adults regain or maintain their independence after illness, injury, or functional decline. Our clinicians work in collaboration with individuals to rebuild strength, confidence, and daily living skills through structured therapy and goal-focused programs. This approach supports ongoing independence, helping clients remain active, reduce reliance on care, and living safely at home.

HOW TO REFER
At First Choice Allied Health, we aim to make the referral process as simple, responsive and easy to navigate as possible, for clients, families and other health care providers.
If you would like to work with First Choice Allied Health, please forward your Service Agreement to (insert email address). Our team will review and return all required supporting documentation promptly.
When you have a specific client in mind, please complete our Referral Form and send it through to (insert email address). Our team will confirm receipt of your referral within 24 hours and begin the intake process for you
Within 2 business days of receiving a referral, our team will make contact with the client to arrange an initial assessment. All appointment details will be emailed to the referring provider. Appointments are usually scheduled within approximately 2 weeks of receiving the referral, pending availability
The initial assessment will be completed at client's home at the scheduled time. This will include a comprehensive review of the referral, subjective and objective assessment, positive, collaborative goal setting, and the development of a tailored management.
Following the initial assessment, a detailed report will be provided to the care partner. This will outline assessment findings, client goals, future recommendations and the proposed management plan. Once this has been approved, ongoing appointments, services and equipment trials can be arranged.
We will maintain regular communication with all relevant care partners following each session, providing updates, recommendations and any escalation of care that may be required.
Services are available to clients with an approved and active Support at Home classification. If you are looking to remain independent at home and would like to access our services, please speak directly to your Support at Home provider and request a referral to First Choice Allied Health
FREQUENTLY ASKED QUESTIONS
Support at Home is an Australian Government program specifically designed to help older people live independently and safely in their own homes for longer. It replaces previous home care programs and provides access to services such as personal care, allied health, assistive technology, and home modifications. Services are arranged through an approved aged care provider following an assessment through My Aged Care.

If you are self-managing your Support at Home services, you may be able to refer directly to us. However, we do require a service agreement to be in place with your registered provider. We are happy to work with you and your provider to arrange the appropriate allied health services.
To access Assistive Technology and Home Modifications (ATHM) funding, you will need to be connected with a registered provider. Once this has been arranged, we can develop a service agreement with the provider and support you in accessing ATHM suitable to your needs.
While our core services are occupational therapy, speech pathology and psychology, we may be able to arrange other services such as physiotherapy and podiatry services where required.
